Incident Form

An incident means that something has happened. It might be the thing that has happened and caused harm, impacted the wellbeing of the person, damage to property or equipment or a situation which is reportable to an external body. An incident has numerous impacted persons - (customer, employee, member of public).

A Hazard is something or a situation that has the potential or could lead to an incident causing harm to a person or damage to property.

A Near miss is something that has happened or a situation which has the potential to or could lead to (but not in this instance) an incident causing harm to a person, impacting the well being of a person or damage to property.

All incident types must be reported as soon as possible, or at the end of your workable shift.
